Print this form, fill it out and send the completed application with a check or money order payable to: North Star Chapter SDC, c/o Henry Votel, 6015 190th St., Forest Lake, MN 55025.

Then, plan to attend Chapter meetings and events. Your newsletter should start arriving the following month.

Membership in the Chapter requires you are a member of the International Studebaker Drivers Club. Chapter events are open to members of any SDC Chapter and Chapter car shows open to all.

Annual North Star Chapter Dues: $15 US

NOTE: When you join and pay for your first membership dues of $15, your second year dues will be prorated based upon the quarter you joined in your first year if you did not join during June-August in the previous year.
